What my wallet card looks like now

Two years ago I took a picture of my wallet card in a very old library book, one which still had the alpha-numberic phone number. I recently noticed the same thing in a book from a different library, so here's the wallet card today:
There is a Franklin Avenue in Hewlett, which is I guess why they were FR4-1667.

    According to that library book sleeve, your wallet card may not be renewed.
